Line 19 of the medical application asks, “Have you visited any health professionals within the last 3 years?” Three years is a long time, and we have had members “run out of room” on the electronic form. The form is limited to 20 visits, so for most of us that should provide plenty of real estate on the form.

Keep in mind that you can combine and report just once any multiple visits to the same health care provider for the same condition. Routine visits to your primary care physician, or follow up visits to specialists post-treatment are two situations where you might have more than one visit. If you have more visits to different doctors than what the form allows, you can include an extra page listing the other visits and provide that to your AME when you have your flight physical.
